Mild puffiness on the forehead is a normal part of healing — here is exactly what to expect.
Mild facial swelling after hair transplant is a normal, harmless part of healing that most patients notice during the first week. It happens because the fluid used during surgery slowly drains downward from your scalp to your forehead and, in some cases, around the eyes. In almost every situation it settles on its own within about seven days and has no effect whatsoever on your final result.
At EsteGo Clinic in Istanbul (Mecidiyeköy, Şişli), our team has guided more than 40,000 patients from over 47 countries through recovery, and forehead puffiness is one of the most common questions we hear in the days after a procedure. This guide explains why it happens, how long it lasts, how to keep it to a minimum, and the rare signs that mean you should contact your clinic.

During your operation, the surgeon injects a large volume of fluid into the scalp. This is a mix of local (tumescent) anaesthesia and sterile saline that numbs the area, protects the follicles, and makes graft placement more precise. Your body simply cannot absorb all of that fluid at once.
Because of gravity, the leftover fluid gradually seeps downward through the loose tissue of the forehead over the next day or two. This is exactly why facial swelling after hair transplant tends to appear on the forehead and around the eyes rather than at the transplanted area itself. It is a mechanical, fluid-related process — not an allergic reaction, and not a sign that anything has gone wrong.
For most people, facial swelling after hair transplant first appears on day 2 to day 4, peaks around day 3 or 4, and then fades quickly. By the end of the first week it has almost always resolved completely.
The swelling can briefly make the forehead feel tight or cause the eyelids to look puffy first thing in the morning. This can be surprising if you are not expecting it, but it is temporary and painless. If puffiness reaches the eye area, it usually clears within one to two days as the fluid is reabsorbed. You can follow the full recovery process in our hair transplant timeline.

You cannot prevent swelling entirely, but a few habits keep it mild. Sleep semi-upright — propped on two or three pillows or in a reclining chair — for the first three to four nights so fluid does not pool in your face overnight. Our team explains the best position in our guide to sleeping after a hair transplant.
If your clinic gives you a forehead headband, wear it exactly as instructed on the days you are told to. A cold compress can help too, but only on the forehead — never place anything directly on the grafts, as pressure can dislodge them in the first days. Staying well hydrated, avoiding alcohol, and not bending forward or lifting heavy objects early on all help reduce puffiness. The American Academy of Dermatology offers general guidance on protecting healing skin after any scalp procedure.

Ordinary swelling is soft, symmetrical, painless, and clearly improving by day five. A small number of symptoms are different and should prompt you to contact your clinic straight away rather than wait.
Be cautious if you notice spreading redness, warmth, or pus around the incision sites, a fever, throbbing pain, or heavy one-sided swelling that keeps getting worse after day four. These can be early signs of infection rather than routine fluid buildup. Our guide on hair transplant infection explains the warning signs in detail. When in doubt, a quick message with a photo settles the question fast.
No. Facial swelling after hair transplant is a surface, fluid-related event and has no impact on graft survival or final density. The transplanted follicles sit securely in the scalp and are not affected by fluid draining across the forehead.
