Go to an emergency department now — not a dental appointment tomorrow — if you have any of: difficulty breathing or swallowing; swelling under the tongue or in the floor of the mouth; swelling that closes the eye or spreads down the neck; inability to open your mouth; fever with rapid heartbeat or confusion.

A dental abscess is a bacterial infection contained under pressure. When it escapes into the tissue spaces of the face and neck, it becomes life-threatening. This is uncommon — and it is why the signs above are non-negotiable.

What an Abscess Actually Is

Bacteria reach the pulp through deep decay, a crack or a failed filling, the nerve dies, and pus collects at the root tip (a periapical abscess). A periodontal abscess instead forms in a deep gum pocket beside the root.

The distinction matters because the treatment differs: one needs the inside of the tooth cleaned, the other needs the pocket drained. Both need the source removed. See tooth decay and gum disease.

Why Antibiotics Alone Do Not Cure It

Antibiotics reduce the surrounding infection but cannot sterilise a dead pulp or an infected root canal — there is no blood supply left to carry them there. If the source is not treated, the abscess returns as soon as the course ends.

Definitive treatment is drainage plus removal of the cause: root canal treatment, or extraction where the tooth cannot be saved. Antibiotics are an adjunct, not the treatment.

The Dangerous Relief

When an abscess bursts, the pressure falls and the pain often disappears almost completely. Many people conclude it has healed. It has not — the infection has simply found an exit, sometimes through a small pimple on the gum (a sinus tract) that drains continuously.

Pain returning after a pain-free interval, or a recurring gum boil, means the source is still there and bone is still being destroyed. Do not wait for pain to decide.

What You Can Do Until You Are Seen

Take over-the-counter painkillers as directed on the packet. Rinse gently with warm salt water. Keep your head elevated when lying down. Avoid heat on the face, which encourages spread.

Do not attempt to lance or squeeze it. Do not take antibiotics left over from a previous illness. And do not rely on painkillers to postpone treatment — they mask progression, they do not stop it. See abscess overview and root canal vs extraction.
