The short answer: yes. Three things account for almost every disappointing decade-old result. First, native hair keeps thinning. The transplanted hairline may be perfect, but if untreated hair behind it continues to recede, a gap or “island” effect can appear between the grafts and the retreating native hair.
Second, a modest decline in density is normal. Ageing affects all hair — transplanted or not. Hair shafts become slightly finer in your 50s and 60s, so a result that was very dense at year two may look a touch softer at year twelve. This is a gradual, natural change, not graft failure.
Third, old operations with aggressive design age worst. A low, straight hairline drawn on a 25-year-old can look unnatural on the same man at 40, and outdated large-punch techniques left visible “pluggy” rows. Because the biggest long-term risk is progression of native hair loss, medication is the quiet hero of every good 10-year result. Finasteride lowers DHT levels and, in most men, halts or dramatically slows further loss; minoxidil improves blood flow and thickens miniaturising hairs.
