The most painful moment is the anaesthetic injection – a few minutes, eased by a fine needle and vibration distraction. After that the surgeon works on a fully numb scalp: you feel pressure, tugging, vibration, no pain. The procedure runs 6–9 hours under local anaesthetic; the commonest complaint on the day is boredom and stiffness, not pain.

The donor area usually aches more than the recipient area. See aftercare.
Normal pain decreases. Not normal: increasing, throbbing, one-sided pain with redness, warmth, pus or fever. That can be infection or folliculitis – rare but treatable. Report it promptly. See safety.
