The connector between your implant and crown — what it is and why it matters.
The abutment is the small connector that links the implant in your jaw to the visible crown, bridge or denture above.

Stock or custom abutments are used depending on the case to ensure a precise, natural emergence profile. Part of every implant treatment.
A well-designed abutment supports gum health and a natural-looking crown.
An implant has three parts. The fixture is the titanium screw in the bone. The crown is the visible tooth. Between them sits the abutment — the connector that supports the crown and shapes the gum around it.
It is the smallest part, the cheapest, and the one that determines whether a dentist anywhere in the world can service your implant in fifteen years.
A stock abutment is prefabricated in standard shapes. A custom abutment is milled for your tooth, from a digital scan, so the crown emerges from the gum at the correct angle with the right contour.
On a back tooth, stock is often perfectly adequate. On an upper front tooth, the emergence profile is the aesthetics — and custom is usually worth it. Ask which you are getting.
Titanium: strongest, the long-term standard, and the safest choice under load. Its grey colour can show through thin gum tissue at the front.
Zirconia: tooth-coloured, better where the gum is thin and translucent, but more brittle. Many clinicians prefer a titanium base with a zirconia superstructure — strength at the connection, aesthetics above it.
The abutment screw is tightened to a specific torque. Chewing, and particularly bruxism, can loosen it. You feel a slight movement of the crown and assume the implant has failed.
Usually it has not. A loose screw is a straightforward repair — retighten to torque, sometimes replace the screw. A loose fixture is failure. A radiograph distinguishes the two in minutes, and the difference between them is a screwdriver versus surgery. See the signs of a failing implant.
Abutments are not interchangeable between systems. The connection geometry is proprietary. If your implant is an unbranded or discontinued system, your dentist at home may be unable to order a replacement abutment or screw at all — and an otherwise healthy implant may have to be removed.
