Protect the Clot: The First 24 Hours
Everything that goes wrong after an extraction goes wrong because the blood clot was lost. For twenty-four hours: no rinsing, no spitting, no straws, no smoking, no vigorous exercise. Bite firmly on the gauze for the period your dentist specifies.
Keep your head elevated, apply cold packs in twenty-minute cycles, and expect some oozing on the first day. If bleeding is heavy and does not slow with firm pressure for twenty minutes, contact your dentist.
Why Straws and Cigarettes Are Genuinely Dangerous
Both create suction. Suction dislodges the clot, exposing bone — dry socket, which affects roughly 2–5% of routine extractions and 20–35% after impacted lower wisdom teeth. Nicotine also impairs the healing itself.

Eating, Cleaning, Rinsing
Day 1: cool, soft food. Nothing hot, nothing to chew on that side. No rinsing at all.
From day 2: gentle warm salt-water rinses after meals if your dentist advises — let the water fall out of your mouth rather than spitting. Brush your other teeth normally; avoid the socket itself.
Week 1–2: reintroduce firmer food gradually. Avoid seeds, nuts and crisps, which lodge in the socket.
Normal Healing Versus a Warning Sign
Normal: discomfort peaking in the first 24–48 hours and then steadily improving; swelling peaking on days 2–3; mild bruising; a slightly unpleasant taste for a day or two.
Not normal: pain that improves and then abruptly worsens around day three; pain unrelieved by painkillers; visible bone in the socket; fever; spreading facial swelling; persistent foul taste. Any of these warrant a call. See swelling after surgery.
Timeline
Soft-tissue closure takes about 1–2 weeks. The bone underneath continues to remodel for 3–6 months, which is why an implant placed later may still require grafting if the site has resorbed. Sutures, if placed, are removed or dissolve within roughly a week.
