Advancement needs a mobile scalp and stable, non-androgenetic hair loss – typically an operation for women with a high but stable forehead. In progressive male loss it is usually contraindicated: if the line moves forward and hair falls behind it, you get a visible problem plus a scar. Transplant is the more flexible but slower route – and the only one in androgenetic loss.
Some centres combine: advancement for density and area, then – months later – single-hair grafts in front of the scar to camouflage it and make the line irregular, therefore natural. A transplant-only lowering typically uses about 1,500–2,500 grafts. See temples and medication vs. transplant.
