What gynecomastia is

Gynecomastia is the benign enlargement of the male breast from glandular tissue, fat or both. It is common and can be hormonal, weight-related, caused by certain medications, or constitutional. The cause should be clarified before any operation.

Surgical options

Liposuction for predominantly fatty forms; gland removal (excision) for firm glandular tissue; usually a combination. The procedure is under general anaesthetic or sedation and takes about 1–2 hours. With good technique the scars are inconspicuous, often at the edge of the areola.

When it is clarified first

With sudden, one-sided or painful enlargement, the cause should be medically clarified (hormones, medication, rarely more serious causes) before operating. Weight loss can also improve a fatty form. An operation treats the symptom, not the cause.

True gynecomastia or pseudogynecomastia

The distinction matters: true gynecomastia involves enlarged firm glandular tissue that does not respond to exercise or weight loss and usually needs excision. Pseudogynecomastia is predominantly fat that responds to weight loss and liposuction. A physical examination and if needed an ultrasound clarify which form is present – and this determines the method. An operation without this clarification may treat the wrong thing.
